How to Book

• Obtain a signed requisition from your referring healthcare provider. As an OHIP-covered service, cardiology consultations or cardiac tests require a signed requisition from a registered Ontario physician or nurse practitioner.
• Have a family doctor? Download our requisition form and visit your family doctor, nurse practitioner or a walk-in clinic.
• We can connect you with a family physician who can assist with a referral to see a PACE cardiologist, if clinically indicated. Download Our New Patient requisition form.

• Ask your healthcare provider to email your signed requisition to info@pace-cardiology.com or fax to 1-855-239-1623.

We will call you directly to confirm your appointment, or we will send the appointment details to the referring healthcare provider. If you would like to reschedule, or if you need to cancel your appointment, please call 1-888-662-0680 (at least 48 hours before your scheduled appointment), or email us at info@pace-cardiology.com.
